In order to provide Notary Service, please ensure that the patient has proper ID (either a California or other U.S. State ID, U.S. Passport or 2 credible witnesses with proper ID who can identify the patient). Also ensure that the patient is aware of what they will be signing, that the person will be awake and alert at the appointment time, and that the person can sign their name.
Before the appointment time, the patient should review and initial every section that requires initials, and complete every section of the document that needs completion, except the final signature. Once in front of the Notary, the patient will sign his/her signature on the document and in the Notary's journal. The Notary will also get a thumbprint from the patient, to go in the journal. If the Notary determines that the person who is signing is not aware of what they are signing, or if a doctor or nurse feels that the person cannot sign the document, then the Notary will not be able to complete the notarization on that day, so it is very important that the person be aware of what they are signing.
